Includes: a focused conversation about your main concern, broad fit, service options, entry costs, and how ongoing recommendations and pricing are determined.
Does not include: evaluation, diagnosis, treatment, imaging interpretation, or medical clearance.
Initial Physical Therapy Evaluation
In-clinic: $200
In-home: varies by location, confirmed before you schedule
May include: complete history, safety review, physical assessment, goal clarification, a working explanation, initial treatment or exercise when appropriate, and a recommendation.
An evaluation does not require you to purchase ongoing care.

A fair question: here’s the honest answer
Different clients require different levels of support. Clinic and in-home settings differ. Some people need only an evaluation or a focused episode of care; others need repeated progression and reassessment.
Publishing a program menu could imply that a visit count should be chosen before clinical fit is understood, which gets the order backwards.
Complete pricing is still provided before any commitment. Nothing is secret, and nothing is negotiable based on who’s asking.
What the price depends on
Your goals and clinical needs, how complex the picture is, what equipment is required, whether care is in clinic or in your home, how often you need to be seen, and how independent you already are between sessions.
You get the complete price before you agree to anything
After a consultation, or an evaluation where that is the better starting point, CrosaCore explains the recommended structure and gives you the full price and payment options in writing. You can ask questions, request alternatives, take time, or decline. Care begins only after you agree.
Where a payment plan applies, you will see the number of payments, the amount of each, and the total. If a plan costs more than paying in full, you will see both figures. Card, HSA and FSA, and financing are all accepted.
No same-day decision is ever required, and the price never changes based on who is asking.
CrosaCore is out of network with insurance plans, so payment is due directly. Superbills are provided on request from the beginning of care rather than only at the end. Reimbursement is never guaranteed, and you are responsible for verifying your own out-of-network benefits.
The policies, stated plainly
- Cancellation and late arrival: paid physical therapy evaluations and follow-up treatment appointments are subject to a strict 24-hour policy. Cancellations inside 24 hours incur a separate $200 fee, and arriving more than 20 minutes late counts the same way. The free consultation carries no fee.
- Refunds: plans paid in full are non-refundable. Unused visits remain yours within the plan period. A documented change in medical status is the one exception, handled as account credit.
- Finishing early: pain resolving does not end your plan. Remaining visits go toward strength, capacity, and return to training. You are never asked to keep reporting symptoms to justify a session.
- Plan end dates: each plan carries an end date, given to you in writing before you agree to it.
- Pausing care: travel, illness, or a demanding stretch at work can extend that date rather than letting visits expire.
- Payment: due at the time of service unless a plan is agreed in advance. Card, HSA and FSA, and financing accepted.
You do not need to make a same-day decision.
The recommended plan should make clinical, practical, and financial sense for you. Questions are welcome, pressure isn’t part of the process.
